EXHIBIT A <br /> UTILITIES - WATER, RECYCLED WATER, SEWER AND STORM <br /> ELECTRICAL <br /> SCOPE OF SERVICES <br /> The City is requesting an experienced licensed electrical contractor to perform routine and emergency <br /> electrical services for the Utilities Division. This work would be performed at any of the City's water, <br /> recycled water, sewer or stormwater facilities located throughout the City. <br /> All contractors' employees shall comply with all Federal, State, and local laws and regulations governing <br /> the installation and testing of electrical equipment and components. All contractors' employees shall <br /> comply with all OSHA and City policies regarding the proper use of Lockout/Tagout procedures. <br /> The following outline provides a scope of work for the City's expectations regarding"as needed"electrical <br /> work for Utilities Division: <br /> 1. Provide electrical troubleshooting, repair, material procurement, and installation services at all <br /> water, recycled water, sewer, and storm facilities including pump stations, reservoirs, and utility <br /> vaults as directed by the City. <br /> 2. Provide electrical emergency services on 24 hour / 7 days a week call out basis for these same <br /> facilities. <br /> 3. Electrical systems/equipment requiring services may include, but are not limited to, 480V <br /> switchgear and motor control centers, 120V lighting panels, control panels, equipment drives and <br /> motors (ranging in size from 2 to 450 horsepower), and ductbanks/wiring systems. <br /> RATE SHEET <br /> The cost quoted below shall include all taxes and all other charges, including travel expenses, and is the <br /> cost the City will pay for the term of any contract that is a result of this bid. <br /> Bidder hereby certifies to City that all representations, certifications, and statements made by Bidder, as <br /> set forth in this Bid Form and attachments are true and correct and are made under penalty of perjury <br /> pursuant to the laws of California. <br /> Contractor's Labor Rates (Hourly) <br /> Position Description Regular Time Overtime Emergency Callout <br /> Foreperson, Electrical $ $ $ <br /> Other: $ $ $ <br /> Other: $ $ $ <br /> Other: $ $ $ <br /> Sample Standard Service Agreement <br /> Page 7 of 8 <br />
