twenty-five dollars ($25) per month toward the RHSP. The total monthly <br /> contribution to the RHSP will be one hundred dollars ($100). <br /> Employees have a vested right to the funds at the time of deposit. Funds <br /> deposited in the RHSP are portable and belong to the employee, regardless of <br /> length of service or reason for separation. <br /> 5.4 Dental <br /> The City shall contribute an amount necessary to provide the current dental care <br /> benefits with an annual maximum of $2000 for individual employees and eligible <br /> dependents. The City shall provide a lifetime maximum orthodontia coverage of <br /> $2,000 for individual employees and eligible dependents. <br /> 5.5 Life Insurance <br /> The City shall contribute an amount necessary to provide coverage equal to twice the <br /> employee's annual salary up to a maximum of $100,000 life and $100,000 accidental <br /> death and dismemberment insurance. For purposes of this benefit, the City's <br /> contribution is not prorated for part-time regular employees. <br /> Employees may purchase, at the employees' own expense and within the limits set by <br /> the insurance carrier, additional amounts of life insurance under the existing policy. <br /> 5.6 Short-Term/Long-Term Disability Insurance <br /> The City shall provide short-term/long-term disability insurance coverage at the <br /> employee's expense for management employees and at the City's expense for <br /> confidential employees. <br /> There is a 14-day waiting period for short-term disability and a 6—month waiting period <br /> for long-term disability. <br /> The maximum short-term disability benefit is $2,500 per week for management and <br /> $1 ,500 per week for confidential employees. <br /> Page 14 of 44 <br />
