Federal or State Legislation <br /> The City reserves the right to meet and confer with the Union for the purposes of coordinating <br /> retired employee's hospital and medical care in the event Federal or State legislation is <br /> implemented that effects retiree's medical insurance. <br /> 16.3 Dental <br /> The City shall contribute an amount necessary to provide dental care benefits for the individual employees <br /> and eligible dependents. The City shall provide a lifetime maximum orthodontia coverage of$2,000 for <br /> individual employees and eligible dependents. <br /> 16.4 Life Insurance <br /> The City shall contribute an amount necessary to provide $50,000 life and $50,000 accidental death and <br /> dismemberment insurance coverage for individual full-time employees. <br /> Employees may purchase,at their own expense and within the limits set by the insurance carrier,additional <br /> amounts of life insurance under the existing policy. <br /> 163 Vision Care <br /> A vision care plan will be provided as an option for employees and the cost of such coverage shall be <br /> included in the 125 Plan(see Section 16.6). <br /> 16.6 Cafeteria Plan <br /> An Internal Revenue Code Section 125 Cafeteria Plan is provided to each full-time employee who is <br /> eligible to enroll in one of the medical insurance plans offered by the City but elects to waive participation <br /> in such medical plans and/or the City's vision care plan. Proof of alternative medical coverage is required <br /> for an employee to waive participation in the City's medical plans. <br /> The 125 Cafeteria Plan affords employees an opportunity to obtain plan credits for the purpose of: (1) <br /> purchasing supplemental benefits; (2)redirecting credits to a flexible spending account; or(3)cashing out <br /> credits at a rate of $0.35 on the dollar. Plan credits are based on the combined sum of the monthly <br /> premiums for Kaiser employee-only medical coverage and the vision care plan at family level coverage <br /> adjusted annually on a dollar-for-dollar basis to match any changes in the monthly premiums. The <br /> maximum contribution limit for medical reimbursement under the Section 125 Cafeteria Plan is $2,650 <br /> per year. <br /> In the event an individual employee loses their primary coverage,the City will allow the employee to re- <br /> enroll in the City's health plans within thirty(30)days of receiving notice that primary coverage has been <br /> lost,thereby enabling the continuation of benefits to the employee. <br /> The City will consider adding supplemental insurance products to the Cafeteria Plan to the extent such <br /> benefits can be offered within an IRC 125 Plan on a pre-tax basis. <br /> 28 <br />
