had such license revoked or suspended, the reason there-- <br />for, and the business activity or occupation of applicant <br />subsequent to such action or suspension or revocation. <br /> <br />for. <br /> <br /> (h) The applicant must furnish a diploma or certifi- <br />cate of graduation from a State approved school wherein <br />the method, profession and work of massage is taught. The <br />term "State approved school" shall mean and include any <br />school or institution of learning which has for its purpose <br />the teaching of the theory, method, profession or work of <br />massage which has been certified by the State of California; <br />Department of Education, Bureau of School Approvals. <br /> <br /> The Chief of Police shall have a right to confirm the <br />fact that the applicant has actually attended classes in <br />a State approved school. <br /> <br /> (i) Such other identification and information necessary <br />to discover the truth of the matters hereinbefore specified <br />as required to be set forth in the application. <br /> <br /> (j) Nothing contained herein shall be construed to <br />deny to the Chief of Police the right to take the finger- <br />prints and additional photographs of the applicant, nor <br />shall anything contained herein be construed to deny the <br />right of said Chief of Police to confirm the height and <br />weight of the applicant. <br /> <br /> (k) If the applicant is a corporation, the name of the <br />corporation shall be set forth exactly as shown in its arti-- <br />cles of incorporation; the names and residence addresses of <br />each officers, directors and each stockholder owning more <br />than ten percent (10%) of the stock of the corporation. If <br />the applicant is a partnership, the application shall set <br />forth the name and residence address of each of the partners, <br />including limited partners. If one or more of the partners i <br />a corporation, the provisions of this section pertaining to <br />a corporate applicant apply. A corporation of partnership <br />shall be deemed to have complied with the provisions of <br />Subsection (h) of this section if the managing director of <br />partner or managing employee of the business has the required <br />diploma or certificate of graduation. <br /> <br />Sec. 4-2.1405. Corporate Applicants. Exemption. <br /> <br /> The provisions of Section 4-2.1404(a) , (b) , (c) , (d) , <br />and (g) , entitled "Application for Permit" relating to <br />requirements for corporate applicants shall not apply to ~ <br />any of the following: <br /> <br /> (a) A corporation, the stock of which is listed on a <br />stock exchange in the State of California or in the City of <br />New York, State of New York. <br /> <br /> (b) A bank, trust company, financial institution or <br />title company to which application is made or to whom a <br />license is issued in a fiduciary capacity. <br /> <br /> <br />
