EXHIBIT A <br />CONDITIONS OF APPROVAL <br />PCUP-255, USA Wu Chi Kung Fu Academy <br />425 Boulder Court, Suites 200 and 300 <br />October 14, 2009 <br />1. The location and operation of the proposed use shall conform substantially to <br />Exhibit B (site plan, floor plans, elevations, and written narrative), dated <br />"Received, September 21, 2009," on file with the Planning Division, except as <br />modified by the following conditions of approval. Minor changes to the <br />approved operation and schedule may be approved by the Director of <br />Community Development if found to be in substantial compliance with the <br />proposal. <br />2. Students 12 years and under in age shall be signed in/out by their <br />parent/guardian. <br />3. Roll -up doors shall remain closed during classes and when children are <br />on-site. <br />4. The operation shall generally adhere to the schedule of activities, dated <br />"Received, September 21, 2009" found in Exhibit B. <br />Hours of operation: Monday — Friday noon — 9:00 p.m. <br />Weekends 9:00 a.m. — 5:00 p.m. <br />The facility shall be allowed to open/close 30 minutes prior to and after <br />sessions for setup and cleanup. <br />5. If additional hours and activities beyond what was stated in the applicant's <br />written narrative and schedule of activities, dated "Received September 21, <br />2009," on file in the Planning Division, are desired, prior City review and <br />approval is required. Such modification may be approved by the Director of <br />Community Development if found to be in substantial conformance with the <br />approval. The Director of Community Development may also refer the matter <br />to the Planning Commission if the proposed changes would be significant. <br />6. If the operation of this use results in conflicts pertaining to parking, interior <br />noise, traffic/circulation, or other factors, at the discretion of the Director of <br />Community Development, this conditional use permit may be submitted to the <br />Planning Commission for their subsequent review at a public hearing. If <br />necessary, the Planning Commission may modify or add conditions of <br />approval to mitigate such impacts, or may revoke the said conditional use <br />permit approval.
