City of Pleasanton "As Needed Services" <br /> Bid#OSD-I 7-02-A NCS <br /> BUILDING MAINTENANCE <br /> STREET MEDIAN MULCH INSTALLATION <br /> SCOPE OF SERVICES <br /> Transport and Install City supplied wood chip mulch in existing street median landscapes. Wood mulch <br /> is available from the City Operations Center. Contractor will be responsible for loading, transporting <br /> and installing wood mulch to a depth of 5 inches. Contractor will also be responsible for traffic control <br /> and median work site clean-up on a daily basis. Scale of operation shall insure a minimum of 50 cubic <br /> yards per work day will be installed. Unit bid should take into account varying median size and design; <br /> and the resulting impact on installation costs and difficulty of installation. <br /> BID FORM <br /> The cost quoted below shall include all taxes and all other charges, including travel expenses. and is the <br /> cost the City will pay for the term of any contract that is a result of this bid. <br /> Bidder hereby certifies to City that all representations, certifications, and statements made by Bidder, as <br /> set forth in this Bid Form and attachments are true and correct and are made under penalty of perjury <br /> pursuant to the laws of California. <br /> Contractor's Rate <br /> Rate per <br /> Scope of Work Cubic Yard <br /> Load, transport and install a minimum of 50 Cubic Yards per day $ 90.00 <br /> Percent of mark-up on material: 15 % Percent Mark-up on invoicing must be demonstrated as a <br /> percentage. Example: Material $1,000 + 10%($100)= $1,100. <br /> Percent of mark-up on outside equipment: 15 % (equipment pre-approved by the City). Mark-up on <br /> invoicing must be demonstrated as a percentage.
