RECOMMENDATION: Scrape and treat the infected wood members with (TIM-BOR) a registered fungicide in accordance with the manufacturers label. Note this fungicide is restricted use product and must be used by a state certified applicator if certification is required. Any restricted use Chemical must be reported to the County Department of Agriculture on their reporting form. This is a Section 1 Item Other Findings: ITEM 4A Cellulose Debris (wood,paper products) was noted in the subarea. Recommendation: Remove the cellulose debris of a rakable size from the subarea and dispose of. This is a Section 2 Item ITEM 4B Cellulose debris, form boards/stakes were noted in the subarea. RECOMMENDATION: Remove the cellulose debris of a rakable size and the form boards/stakes from the subarea. If it is found that it is impractical to remove the boards/stakes, treat the embedded wood with (Tim-Bor) an approved fungicide, in accordance with the manufacturers label. NOTE: IF IT IS FOUND THAT DURING THE REMOVAL OF THE DEBRIS AND/OR BOARDS/STAKES THAT THE WOOD IS INFESTED WITH SUBTERRANEAN TERMITES, A SUPPLEMENTAL REPORT WILL BE ISSUED AND ANY FINDINGS AND ADDITION CHARGES WILL BE LISTED. ****** This is a Section 2 Item ****** ITEM 4C There is earth to wood contact at the foundation in the subarea.
