The HSC reviewed applications through the online grants management system and
evaluated projects against the funding priorities it adopted in November 2015 (based on
the 2011 Tri- Valley Human Services Needs Assessment and 2013 Strategic Plan). The
2016 HSC funding priorities are as follows:
e Priority Needs — Projects that address the following populations and /or program
areas: (Listed in Alphabetical Order)
o Disability Services and Access
o Food and Nutrition
o Health Care and Behavioral Health
• Senior Services
• Workforce Development/Job Training
o Youth Services
e Service Delivery with an emphasis in the following areas:
• Coordination and collaboration with other service providers
• Prevention (activities that help prevent a problem from emerging)
• Self Sufficiency (activities that promote self - reliance and are aimed at
reducing dependency on assistance programs and services)
o Transportation consideration (applications that incorporate or anticipate how
clients will access the proposed programs or activities)
Applications were scored and ranked by the HSC in the online system. Staff then used
those scores as the basis for developing an initial funding recommendation. On March
2, the HSC held a hearing for grant applicants during which they heard presentations
from new applicants /projects and from other applicants prior to their funding
deliberation. Below is a summary of the programs reviewed by the HSC and their
funding recommendations.
Table 4
CDBG and General Fund (Human Services) Recommendations
Agency
Project Description
Rec
Funds
Source of
Funds
Abode Services
The Hope Street Project provides street outreach services to
$10,000
Gen Fund
homeless, extremely vulnerable, and currently disconnected
(HSC)
from services.
