JOINT MEETING PPP <br /> HFAUNION UNIFIED SCHOOL DISDJR <br /> April 27, 2015 <br /> TITLE: PLEASANTON HEALTH INITIATIVE -TRI-VALLEY HEALTH FAIR IN PLEASANTON <br /> RECOMMENDATION <br /> Receive information regarding the Pleasanton Health Initiative and the Tri-Valley Health <br /> Fair in Pleasanton. <br /> SUMMARY <br /> The 2011 Eastern Alameda County Human Services Needs Assessment: Findings Report <br /> indicated the need to ensure access to health related services and to embrace the growing <br /> diversity in the region. The Pleasanton Health Initiative was designed as a joint project <br /> between the City of Pleasanton and the Pleasanton Unified School District (PUSD) to <br /> address this need. As one approach to meet this need, the Health Fair serves as a gateway <br /> to make contact and engage especially the underserved communities to provide health <br /> screening, linkage, and healthcare enrollment to 200 families in Pleasanton. <br /> BACKGROUND <br /> The City of Pleasanton and Pleasanton Unified School District participate together in the <br /> Alameda County Tri-Valley Youth and Family Opportunity Initiative which is funded <br /> through Measure A. The goal of the initiative is to strengthen the capacity of the Tri-Valley <br /> region to provide a continuum of high quality, accessible school-linked health and <br /> wellness support to youth and families experiencing poor health and educational <br /> outcomes. The initiative is based on evidence that there is a critical link between children's <br /> healthy development and educational attainment, ultimately impacting long term health <br /> outcomes. Additionally, it is recognized that families are the most fundamental factor <br /> influencing the lives and outcomes of children and that supported and supportive families <br /> are critical to reversing health disparities. <br /> The Pleasanton Health Initiative has been granted $16,666 from Measure A, a portion of <br /> which will cover the costs of the Health Fair. <br /> DISCUSSION <br /> Planning for the Health Fair is underway. City and PUSD staff convened a focus group of <br /> parents and teens from the community we are seeking to serve; several meetings have <br /> been held to elicit ideas of what the community would like provided at the event. <br /> The focus group has requested that both children and adults be able to participate in the <br /> services to be provided. Services and information requested by the families include <br /> immunizations, medical and dental screenings, mental health screening, nutrition <br />
