Describe consultation with the Continuum(s)of Care that serves the jurisdiction's area in <br /> determining how to allocate ESG funds, develop performance standards and evaluate <br /> outcomes,and develop funding, policies and procedures for the administration of HMIS <br /> Consultation with Every One Home,the Alameda Countywide Continuum of Care,on the use of <br /> Emergency Solutions Grant(ESE)funds, began in early 2012,when representatives from the City of <br /> Berkeley,the City of Oakland, Alameda County Housing and Community Development Department <br /> (Urban County grantee),and EveryOne Home worked together to implement the new ESG requirements <br /> in a way that would be consistent county-wide and would continue a collaboration which began in 2009 <br /> with American Recovery and Reinvestment Act (ARRA) Homelessness Prevention and Rapid Re-housing <br /> (HPRP)funds. This collaboration resulted in the creation of Priority Home Partnership(PHP),which was <br /> a single county-wide program to implement HPRP. EveryOne Home held a community-wide meeting at <br /> which additional consultation and public input into the use of ESG funds was solicited. A'series of <br /> meetings with EveryOne Home and the ESG grantees continues through the year and a coordinated ESG <br /> program was established and began implementation in early 2013. This coordinated program will use <br /> this same structure for FY15-FY19 ESG funding. <br /> V <br /> • Alameda County HCD,City of Berkeley and City of Oakland met to discuss regional issues affecting <br /> all three entitlement jurisdictions and to coordinate HOPWA and,Continuum of Care consultations. <br /> • Presentation of the draft Community Development Block Grant Unincorporated County Strategy <br /> 2015-2019 before the Unincorporated Services Committee of the Alameda County Board of <br /> Supervisors. Received Citizen input on Strategy to date. <br /> • Meeting between Healthy Homes and Urban County to develop strategies for ways the Healthy <br /> Homes program could meet the needs of the Urban County jurisdictions for the next five years. <br /> • Email and telephone conversations regarding needs and activities for next five years. Also consulted <br /> with City of Alameda Housing Authority and City of Livermore Housing Authority. City of Pleasanton <br /> Housing Authority is a part of the Housing Authority of Alameda County. <br /> • Stakeholders survey for the Analysis of Impediments to Fair Housing Choice-telephone and email <br /> survey. N` <br /> • Survey sent out to HOME Consortium jurisdictions Citizen's Advisory Committees(or equivalent)to <br /> get their perceptions on what the priority needs are for the HOME Consortium over the next five <br /> years. 143 responses received. Information used to help shape priority areas. <br /> • Joint meeting with the Cities'of Berkeley and Oakland, HCD and Everyone Home to develop five year <br /> strategies for ending homelessness. <br /> • Meeting with the Cities of'Oakland, Berkeley, HCD and Contra Costa County to discuss regional <br /> HOPWA needs for the next five years. <br /> Efforts were made to include as broad a group of community stakeholders as possible.
