inspection of non-compliant electrical mast arms and a replacement source for same. CM, Inc. continues <br /> to provide quality assurance testing and source inspection in accordance with Caltrans requirements as <br /> required by City's agreement with the State. Note our testing budget was based on City's initial <br /> construction estimate of$3.6M for the project while actual low bid was$5.3M. <br /> CM, Inc. has attempted to mitigate the above impacts by cutting back on resident engineer/project <br /> manager hours; changing staff shifts to adjust to contractor's changing work schedule to minimize staff <br /> time; not backfilling open positions due to staff time off; and meeting minimum quality assurance testing <br /> requirements. In contrast,the number of force account change orders with the contractor;additional <br /> review times for complicated value engineering proposals;additional effort to resolve SWPPP risk level <br /> and traffic controller testing issues; additional effort required due to contractor's repeated change of <br /> contract personnel; development of a source inspection quality manual required by Caltrans, but not <br /> included in our scope; and time extensions have hampered further efforts to mitigate impacts. The sum of <br /> these efforts has resulted in construction management and inspection hours billed to date being roughly <br /> in-line with those originally estimated for a corresponding period for the I-580/Poothill Road project, <br /> despite efficiencies lost with delays on the I-680Bernal Avenue project. Quality assurance testing and <br /> source inspection costs remain impacted. <br /> We are requesting the City issue a contract amendment in the amount of 587,000.00 to cover additional <br /> construction management, inspection, and quality assurance testing costs incurred due to the above listed <br /> reasons.The breakdown of this estimate is as follows: <br /> Hours Rate Extended <br /> Bill Carlson, PE Project Manager/ 240 $166.86 $40,046.40 <br /> Resident Engineer <br /> Mimi Lee, PE Construction Insp/ 180 $131.69 $23,704.20 <br /> Office Engineer <br /> Testing Services $23,000.00 <br /> TOTAL $86,750.60 <br /> Say $87,000.00 <br /> This amount would cover costs until contractor's projected finish date plus an additional week of field <br /> work as I feel their schedule is aggressive.
