This project is slated for completion in 2014-15. Upon completion, the new site will <br /> include 27 examination rooms, counseling rooms, a CLIA Level One laboratory, a draw <br /> station, a pharmacy and administrative space (see floor plan below). The clinical areas <br /> will also include seven mental health counseling rooms which will accommodate Axis's <br /> integrated behavioral health services. Upon completion, Axis will nearly double its <br /> capacity, and will be able to serve a projected 20,000 patients. Even at full build-out, <br /> Axis will still be stretched to meet the community's needs. Architectural <br /> design is being completed, a general contractor has been selected, and a capital <br /> campaign has been initiated. The current project cost estimate is approximately <br /> $12 million, and project support is expected to include a combination of bond financing, <br /> local and county support and capital campaign proceeds. A preliminary summary of <br /> sources and uses is shown below. The proposed Section 108 loan (discussed. in the <br /> following section) would reduce the amount of bond financing necessary and, therefore, <br /> the amount of long-term agency debt. <br /> Page 5 of 9 <br />
