5. Benefits for Medicare Eligible Retirees <br /> Retiree health benefits will cease once the employee becomes Medicare eligible. <br /> 6. Retiree Health Savings Account <br /> PCEA/AFSCME and the City will meet within ninety (90) days after adoption of this <br /> agreement to review and select a Retiree Health Savings Account (RHSA). For <br /> employees hired on or after June 1, 2011 and who successfully pass probation, the City <br /> will contribute thirty-five dollars ($35) per pay period to the RHSA beginning the first <br /> pay period after the RHSA has been selected. Additionally, each employee eligible for <br /> the City contribution will have twelve dollars ($12) per pay period deducted pre-tax as an <br /> additional employee contribution toward the RHSA. The total contribution to the RHSA <br /> will be forty-seven dollars ($47) per pay period. <br /> 15.3 Dental <br /> The City shall contribute an amount necessary to provide dental care benefits for the <br /> individual employees and eligible dependents. The City shall provide a lifetime <br /> maximum orthodontia coverage of $2,000 for individual employees and eligible <br /> dependents. <br /> • The maximum contribution shall be set at $145.00 per month. <br /> Should the costs of the dental plan exceed $145 per month during the term of the <br /> Agreement, the City will pay the difference. <br /> For the purpose of providing aforesaid dental care coverage for eligible regular part-time <br /> employees including job share employees who average between twenty (20) and twenty- <br /> nine (29) hours per week, the City shall contribute an amount not to exceed fifty percent <br /> (50%) of the city's monthly contribution established for full time employees as set forth <br /> above. For regular part-time employees including job share employees who average <br /> between thirty (30) and thirty-nine (39) hours per week, the City shall contribute an <br /> amount not to exceed seventy-five percent (75%) of the City's monthly contribution <br /> established for full-time employees as set forth above. <br /> 15.4 Life Insurance <br /> The City shall contribute an amount necessary to provide $35,000 life and $35,000 <br /> accidental death and dismemberment insurance coverage for the individual full-time <br /> employee. The City shall contribute an amount necessary to provide $26,250 life and <br /> $26,250 accidental death and dismemberment insurance coverage for the individual <br /> regular part-time employee who averages between thirty (30) and thirty-nine (39) hours <br /> per week; and $17,500 life and $17,500 accidental death and dismemberment insurance <br /> coverage for the individual regular part-time employee who averages between twenty <br /> (20) and twenty-nine (29) hours per week. <br /> Employees may purchase, at the employees' own expense and within the limits set by the <br /> insurance carrier, additional amounts of life insurance under the existing policy. <br /> -26 - <br />
