EXHIBIT "A" <br /> SCOPE OF WORK <br /> LPFD has developed a contracted position within the Fire Department for a licensed <br /> physician (meeting qualifications) to monitor and conduct the Department's Wellness- <br /> Fitness Program as described. Under the direction of the EMS Manager, the Department <br /> Physician will provide services to maintain an effective wellness and fitness program that <br /> encompasses injury prevention, medical, physical, and mental well being. <br /> This Program will provide optimum employee care while being in compliance with State <br /> and Federal requirements and the International Association of Fire Chiefs (IAFC) and <br /> International Association of Firefighters (IAFF) Wellness-Fitness Initiative, 3`d edition <br /> (WFI) (Attachment A). The Department Physician will be involved with the health, wellness, <br /> fitness and safety of all 118 uniformed, safety personnel from the beginning of their career <br /> (initial baseline evaluation), throughout their career (annual fitness/health evaluations and <br /> education), and as retirement approaches. Scope of service will include active participation <br /> with program opportunities, medical counseling and education. Certain services (such as <br /> blood draws and immunizations) will require the vendor to visit the fire station to <br /> accommodate the unique nature of the Firefighter schedule. <br /> Confidentiality of medical information is a critical aspect of medical and fitness evaluations. <br /> The unauthorized release of personal details that may be recorded as part of a medical <br /> evaluation can and does cause legal, ethical, and personal problems either for the employee, <br /> the employer or the examining physician. All information obtained from medical and fitness <br /> evaluations is confidential, and the employer shall only be provided a statement regarding <br /> fitness for duty, necessary work restrictions, and appropriate accommodations. <br /> Scope of work for this project includes the following tasks: <br /> Task 1 - Provide initial baseline medical and fitness evaluations for LPFD uniformed <br /> employees: <br /> Baseline: (New Employees) <br /> 1. Medical History Questionnaire (Conducted by Physician) <br /> The medical history questionnaire is developed to provide baseline information with <br /> which to compare future medical concerns. This questionnaire will be completed with <br /> follow up information on an annual basis, which will focus on changes in health status. <br /> The LPFD physician will work in conjunction with City's contracted medical physician to <br /> obtain medical information from the pre-employment exam. <br /> 2.
