EXHIBIT A <br /> CONDITIONS OF APPROVAL <br /> PUD-65-01M, Nick Kavayiotidis <br /> 5980 and 5998 Sunol Boulevard <br /> October 12, 2011 <br /> PROJECT SPECIFIC CONDITIONS <br /> Planning <br /> 1. Prior to the issuance of a building permit, the approved Lot Line Adjustment (Case <br /> No. PLLA-98) shall be recorded. <br /> 2. The site development standards for the subject properties shall follow the Office District of <br /> the Pleasanton Municipal Code, except as modified by the North Sycamore Specific Plan <br /> and the conditions of approval for Case PUD-65-01 M. <br /> 3. Condition No. 8 of PUD-65 shall be revised to include the proposed memory care/assisted <br /> living facility. The permitted uses for the subject properties shall be subject the following list <br /> of uses with the added use underlined: <br /> o Administrative and Business Offices <br /> o Design-Related Offices <br /> o Insurance Offices <br /> o Investment Services <br /> o Legal Services <br /> o Medical and dental Offices <br /> o Real Estate Offices <br /> o Research Services <br /> o Memory care/assisted living facilities <br /> 4. No on-site improvement and/or grading shall be allowed within the creek setback area in the <br /> northern portion of the project site. <br /> 5. Prior to issuance of a building permit, the applicant shall reimburse the North Sycamore <br /> Specific Plan funding developer(s) for the 5980 Sunol Boulevard property's pro rata share of <br /> the North Sycamore Specific Plan capital improvements as stipulated by the North <br /> Sycamore Specific Plan Finance Plan. <br /> 6. Prior to occupancy, the applicant shall join the North Sycamore Specific Plan Maintenance <br /> Association as determined by the City Engineer. The Maintenance Association fees for this <br /> project shall be based on the 5980 Sunol Boulevard property's pro rata share in a manner <br /> consistent with the North Sycamore Specific Plan Finance Plan. <br /> 7. An acoustic study shall be conducted by a State of California licensed acoustic engineer for <br /> the proposed development. This study shall include mitigation measures to ensure the <br /> proposed development conforms to the multiple-family residential noise criteria as specified <br /> in the Noise Element of the Pleasanton General Plan 2005-2025. This study shall be <br /> included in the construction plan sets submitted for plan check and is subject to the review <br /> Ordinance No. 2023 <br /> Exhibit A Page 1 of 18 <br />
