G. An Operator shall have a Carrier Identification number and lid Motor Carrier Property <br /> (MCP) permit and shall provide copies of these documents to PPD annually and upon <br /> request. <br /> 1) Expiration of an Operator's MCP and/or suspension of the MCP, pursuant to CVC <br /> §34623, shall result in the immediate suspension of the Operator, as well as <br /> additional disciplinary action which may be imposed. <br /> H. An Operator shall, at its sole expense, have its employees participate in an annual <br /> random drug testing program. Participation is subject to verification by the PPD. <br /> 6. BUSINESS RECORDS <br /> A. An Operator shall maintain records of all tow services furnished. The records shall be <br /> maintained at the Operator's place of business. Invoices shall at a minimum include the <br /> Operator's name and address, a description of each vehicle towed, including the name <br /> or make, the motor or other number of the vehicle, the nature of any damage to the <br /> vehicle, the license number and registration number shown by the license plates or <br /> registration card (if either is attached to the latter in a vehicle in a clearly discernible <br /> place), the nature of the service provided, date, start time, end time, location of call, <br /> location where vehicle is towed, itemized costs of towing and storage, the tow truck <br /> driver's name and truck used , the name and address of the person storing the vehicle <br /> or requesting the towing, and the names of the registered owner and driver of the <br /> vehicle (if ascertainable). <br /> a. Upon termination of the storage, a statement shall be added to the <br /> record as to the disposition of the vehicle, including the name and <br /> address of the person to whom the vehicle was released and the date <br /> of such release. <br /> b. If the service provided caused the time to exceed one hour, a clear, <br /> itemized, and detailed explanation of the additional service shall be <br /> documents on the invoice pursuant to CVC §22651.07(e)(7). <br /> B. At an Operator's primary office, Operator shall also maintain business records relating <br /> to personnel, insurance, medical certifications, personnel taxes, payroll, applicable <br /> operating authority, local operating authority, lien sale actions, Federal Communications <br /> Commission licensing (if applicable), and non-PPD tows. <br /> C. An Operator shall keep a written record of every vehicle stored for a period longer than <br /> 12 hours pursuant to CVC§ 10650(a). <br /> D. An Operator shall maintain a current list of drivers and provide it to the PPD annually <br /> and upon request. The list of drivers shall include: <br /> a.
