ATTACHMENT A <br /> (3) For Very Low Income Residents and the Dementia Resident. <br /> eighty percent (80%) of fifty percent (50%) of area median income. <br /> <br /> An assumed household size of one person will be used in setting the Affordable Charges. <br /> <br /> (c) "Assisted Living" shall mean the provision of the following basic services. <br />which shall be provided to all Residents regardless of income level and in accordance with the <br />State of California's licensing requirements for assisted living facilities as determined by the <br />Department of Social Services of the State of California: <br /> <br /> (1) Three (3) meals per day; <br /> <br /> (2) Weekly housekeeping services: <br /> <br /> (3) Linen services: <br /> <br /> (4) Thirty (30) minutes per day per Resident of assistance with <br /> activities of daily living, such as dressing, bathing and grooming: <br /> <br /> (5) Medication management; <br /> (6) Twenty-Four (24) hour staffing: <br /> (7) Emergency Call systems; <br /> (8) Health Monitoring: <br /> (9) Coordinated activity schedule to encourage socialization; and <br /> (10) Weekly transportation services. <br /> <br /> (d) "Authorized Officers" shall meam in the case of the Lessor, the City <br />Manager, or Acting City Manager, or his or her designee, and in the case of the Lessee, the <br />President or any officer. <br /> <br /> (e) "City Regulatory Agreement" shall mean the regulatory agreement for the <br />benefit of the City setting forth the requirement to rent to the Extremely Low Residents, Very <br />Low Income Residents and the Dementia Resident at Affordable Charges. <br /> <br />(f) "Date Of This Lease" shall mean the date first written above on which this <br />Lease is effective. <br /> <br /> (g) "Dementia Resident" shall mean a Primary Resident who has been <br /> medically diagnosed with a form of dementia, such as Alzheimer's such that his or her health or <br /> <br /> 2 <br /> 144\105\153233.13 <br /> <br /> <br />
