Exhibit "A" - SCOPE OF SERVICES <br /> Firehouse Arts Center Donor Recognition Wall <br /> Services to be provided by Jill Turman (the artist) <br /> The Artist shall create a donor recognition wall (the artwork) per approved design documents <br /> in forged steel and aluminum. The artist shall be responsible for delivery and installation of the <br /> artwork. <br /> The artwork shall consist of five main elements: <br /> 1) A grid of brushed aluminum tubing affixed to the wall <br /> 2) Forged steel, textured "swoops" affixed to the aluminum grid on the wall <br /> 3) 45 "display" name plaques in forged steel and engraved aluminum, affixed to the wall* <br /> 4) 45 "gift" name plaques in forged steel and engraved aluminum to be given to donors* <br /> 5) 5 forged plaques, each in a different texture (indicating giving level) to be affixed to the wall <br /> *Additional name plaques shall be furnished by the artist at the request of city for an <br /> additional cost. <br /> Name Plaques: <br /> The artist shall furnish the city with 90 pieces of pre cut aluminum, ready for engraving. <br /> The city shall be responsible for labor and expenses associated with engraving the aluminum <br /> with donor names. <br /> The artist shall furnish the forged steel back plates for the plaques in five different textures to <br /> correspond with giving levels, patina the aluminum engraving, and shall be responsible for <br /> affixing the pre- engraved aluminum to the back plates. <br /> The artist shall be responsible for affixing 45 "display" name plaques to the wall <br /> The artist shall deliver 45 "gift" name plaques with back supports (designed for table top <br /> display) to the city. <br />
