Priority: Build on inter jurisdictional cooperation to achieve housing <br /> and homeless needs. <br /> Priority Analysis and Obstacles to Meeting Underserved Needs <br /> Housing and homeless needs in Alameda County are guided by the Every One Home Plan that was <br /> completed in 2007. The Plan identified gaps in the current service delivery system and is intended to <br /> link homeless policies and plans in each of the three HOME jurisdictions in Alameda County: <br /> Berkeley, Oakland and the Alameda County HOME Consortium. The Plan has lead to the creation <br /> of Consumer Rights Standards, assessment and implantation of Health, Safety, and Accessibility <br /> Standards for shelters, and advocacy for retaining and expanding critical support services and <br /> housing. <br /> Every One Home recently implemented a Homeless Management Information System (HMIS) and <br /> Standards of Service in Alameda County. The HMIS for homeless service providers was developed <br /> and implemented to obtain an unduplicated count of demographics and needs of homeless <br /> individuals and families in Alameda County. The HMIS system allows better tracking and shared <br /> information on individuals while they are in the housing system. The goal is to prevent duplication <br /> of services to clients and allow the client to get the assistance they need depending on where they <br /> stand in the continuum. <br /> Objectives <br /> The objectives presented in this section reflect the broad areas which will be pursued under this <br /> priority.
