As required by Section 6.24.050(B), written proof that the applicant is a <br /> member, in good standing, of a state or national professional association <br /> devoted to the massage specialty and therapeutic enhancement approach. <br /> <br /> 3. All information required by Section 6.24.060(C), except the following: <br /> <br /> (i) each applicant shall provide only the name, street address, and <br /> telephone number of each other business in which the applicant has <br /> been employed within the past seven (7) years along with the dates <br /> of employment, positions held by the applicant, and a contact <br /> person at each location to comply with Section 6.20.060(C)(5); <br /> <br /> (ii) each applicant need not provide information as required in Section <br /> 6.24.060(C) (12), (13), (15), (16), (17), and (20); and <br /> <br /> (iii) each applicant shall provide written authorization for the City, its <br /> agents, and employees to seek information and conduct an <br /> investigation into the truth of the statements set forth in the <br /> application and into the background of the applicant to comply with <br /> Section 6.24.060(C)(22). <br /> <br /> 4.. The full name, street address, and telephone number of each business <br /> premises in which the massage technician administers massage. <br /> <br />6.24.080 Application Review. Upon receipt of an application, the Police Chief shall review <br /> the application and supplementary material. If it is clear from the face of the <br /> application and supplementary material that the applicant is not qualified for the <br /> permit sought or if the required fee has not been paid, the application may be <br /> denied without further investigation. If it appears from the face of the application <br /> and supplementary material that the applicant may be eligible for the permit <br /> sought, the Police Chief shall verify the information submitted by the applicant and <br /> shall further investigate the qualifications of the applicant as follows: <br /> <br /> A. The Police Chief shall photograph the applicant, take a full set of the applicant's <br /> fingerprints, and submit the fingerprints to the Department of Justice for <br /> evaluation. Upon receipt of the report from the Department of Justice, the Police <br /> Chief shall review the criminal history (if any) of the applicant; <br /> <br /> B. The Police Chief may conduct additional investigations in a manner authorized by <br /> law when necessary to determine if the applicant meets the qualifications for a <br /> permit pursuant to this Chapter; and <br /> <br /> <br />
