H. A copy of the certificate of insurance for the business and taxicabs, or a written <br />statement from the insurance company agreeing to provide insurance should the application be <br />granted. This insurance company shall be headquartered in the United States, authorized by the <br />State to provide insurance in this State and approved by the City Attorney as to its financial <br />status. <br /> <br /> I. Copy of the Federal Communication Commission license to operate a base and <br />mobile radio units. <br /> <br /> J. A statement of personal business history regarding taxicab operations/ownership. <br /> <br /> K. A statement as to whether the applicant has ever been denied a municipal taxi <br />permit, or if it was issued whether it was ever suspended or revoked, and if yes, why. <br /> <br /> L. Such other additional information as the Chief of Police may reasonably require. <br /> <br />6.40.030 Procedure on Application and Hearing. <br /> <br /> A. Publication of Application. <br /> <br /> Upon the filing of any application for a Certificate with the Chief of Police, notice <br />thereof shall be published one (1) time in a local newspaper of general circulation. <br /> <br /> B. Protest and Hearing. <br /> <br /> Persons interested or protesting an application should notify the Chief of Police in <br />writing within five (5) business days of the publishing of said notice of application. <br /> <br /> Within thirty (30) days of receipt of a written protest, the Chief of Police shall <br />convene a hearing regarding said application. At the hearing, parties shall not be bound by the <br />formal rules of evidence. The Chief of Police shall hear the information submitted by the <br />protesting party, any interested party, and the applicant. The Chief of Police shall issue a written <br />decision no later than ten (10) business days following the hearing. Such decision shall be <br />supported by the findings set forth in Section 6.40.030.C, below. <br /> <br /> C. Findings of Decision. <br /> <br /> The Chief of Police shall issue or deny such Certificate after investigating the <br />applicant and the facts set forth in the application. If the Chief of Police finds that the applicant <br />is fit, willing and able to meet the provisions of this Chapter and any and all rules promulgated <br /> <br /> Page 5 <br /> <br /> <br />
