14.2 Dental <br /> The City shall contribute an amount necessary to provide dental care benefits under the Ameritas <br /> Plan for the individual employee and eligible dependents; Maximum orthodontia coverage shall <br /> be $2,000 lifetime allowance. <br /> 14.3 Vision <br /> During the term of this Memorandum of Understanding, the City shall contribute the premium <br /> necessary for the VSP Vision Plan family coverage for health insurance participants who are not <br /> covered by Kaiser. <br /> 14.4 Long Term Disability <br /> Effective May 1, 1998, the City's maximum monthly long term disability insurance premium <br /> contribution shall be increased to twenty three dollars ($23.00). The City shall not be obligated to <br /> contribute an amount in excess of twenty three dollars ($23.00) throughout the term of this <br /> agreement. <br /> 14.5 Life, Accidental Death and Dismemberment Insurance <br /> The City shall contribute an amount necessary to provide $35,000 life and $35,000 accidental <br /> death and dismemberment insurance coverage for Association members. The monthly premium <br /> for life insurance coverage shall be paid by the City during the term of this agreement. <br /> Section 15. Retirement <br /> 15.1 Payment of Employee Contribution to PERS <br /> Effective January 1, 1988, the City shall pay the employee's contribution to the Public Employees' <br /> Retirement System (PERS), to include the "Single Highest Year" option. The amount of said City <br /> payment shall equal but not exceed nine percent (9 %) of the employee's compensation subject to <br /> retirement contributions. Effective February 11, 1995, the city shall report as special <br /> compensation the dollar value of the city's payment of the employee's nine percent retirement <br /> contribution. <br /> 15.2 Adoption of PERS Credit for Unused Sick Leave Option <br /> Upon City Council adoption of this memorandum of understanding, the City shall proceed to <br /> amend its contract with the Public Employees' Retirement System for the purpose of implementing <br /> the PERS Credit for Unused Sick Leave Option. Said amendment to be effective as soon after <br /> April 1, 1995 as the contract amendment process can be completed. <br /> 153 Adoption of PERS 3% (7a 50 Retirement Formula <br /> Effective October 1, 2001, the City will implement the PERS 3% @ 50 retirement formula for the <br /> PERS safety members in the Police Department. <br /> 15.4 Adoption of PERS Military Buyback Provision <br /> Effective October 1, 2001, the City will implement the PERS Military Buyback Provision for <br /> PERS safety members in the Police Department. <br /> - 12 - <br />
