An =mployee eligible for t~.~orary disability <br />payments may use accumulated sick leave in order to <br />maintain his/her regular income; provided, however, <br />that the employee shall be allowed a credit against <br />sick leave to the extent that temporary disability <br />payments are retained by the City. <br /> <br />In order to receive compensation while absent on <br />sick leave the employee shall cause notification to <br />his/her immediate supervisor or the Police Chief <br />prior to the time set for beginning his/her daily <br />duties. When the absence is for more than eight <br />(8) hours, the employee may be required to file a <br />physician's certificate or a personal affidavit <br />with the Police Chief stating the cause of the <br />absence. <br /> <br />Section <br />11.1 <br /> <br />11.2 <br /> <br />11. Leaves Of Absence <br /> <br />Any regular or probationary employee may upon written <br />request to the Department Head and the Personnel <br />Director, be granted by the City Manager a leave of <br />absence without pay, for a period not to exceed three (3) <br />months. The written request must state the reasons for <br />the leave. The Department Head may grant a regular or <br />probationaryemployee a leave of absence without pay, for <br />a period not to exceed one (1) calendar week. Said <br />leaves shall be reported to the Personnel Director. <br /> <br />During said leave, an employee shall not accrue vacation <br />or sick leave benefits or be eligible for holidays, nor <br />shall such time be considered as service time. <br /> <br />Extended Medical Leave <br /> <br />~_ny regular or probationary employee who is temporarily <br />disabled from work due to a medical condition may, upon <br />written request to the Department Head and the Personnel <br />Director, be granted by the City Manager, a leave of <br />absence without pay for a period not to exceed three (3) <br />months beyond the exhaustion of any accrued sick and <br />vacation leave. With a recommendation by the Department <br />Head to and approval by the Personnel Director, the City <br />Manager my extend the leave for up to an additional <br />thirty (30) days. In unusual circumstances involving <br />catastrophic illness, the city manager may, upon the <br />recommendation of the department head and personnel <br />director, extend a leave of absence without pay up to but <br />not exceeding one year from the date of disability. <br /> <br />The term medical disability includes all temporary <br />medical disabilities including pregnancy, childbirth and <br />associated medical conditions.
