CITY OF PLEASANTON COUNTY OF ALAMEDA STATE OF CALIFORNIA RESOLUTION NO. 76-39 WHEREAS, of Nebraska a group dental insurance policy; and WHEREAS, the group dental insurance policy was submitted to the City Council at their meeting of February 23, 1976; 'NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF PLEASANTON DOES RESOLVE AS FOLLOWS: APPROVING AUTHORIZATION OF EXECUTION OF AGREEMENT FOR GROUP DENTAL INSURANCE PLAN WITH BANKERS LIFE INSURANCE COMPANY OF NEBRASKA WHEREAS, by Memorandum of Understanding executed in June,1975, the City of Pleasanton and the Pleasanton City Employees Association agreed upon terms and conditions of employment to govern 1975-76 and 1976-77 fiscal years; and ~HEREAS, by terms of said agreement, it was determined that the City would undertake an investigation of dental insurance plans to provide insurance coverage to P.C.E.A, employees and their dependents; and WHEREAS, the City of Pleasanton did, in fact, undertake a study and met with representatives of the P.C.E.A. for the selection of an appropriate plan; and WHEREAS, it was determined that the group dental insurance plan provided by Bankers Life Insurance Company of Nebraska would be sufficient to meet the obligations set forth in the aforementioned Memorandum of Understanding; and WHEREAS, the City Council, at its meeting of December 8, 1975, approved the execution of a preliminary application for enrollment with Bankers Life Insurance Company of Nebraska to provide a dental insurance plan covering the P.C.E.A. employees and their dependents in accordance with the aforementioned current Memorandum of Understanding; and the City has received from Bankers Life Insurance Company Section 1. Authorizes execution of a group dental insurance policy with Bankers Life Insurance Company of Nebraska as attached
